    ​Complete this form before 8 October 2025.

    The form below must be completed and submitted in order to be considered for a committee position. All committee positions are held for 12 months from the date of the Annual General Meeting 15 October 2025. If more nominations than positions available are submitted before the AGM then the nominations will be voted on at the AGM.
    A CV of your relevant experience must also be submitted to be considered for the position/s above selected. 
    To nominate for a committee position, you must be referred.
